چکیده

Non-operative management is currently the standard of care for blunt abdominal trauma patients who sustain solid organ injuries and are hemodynamically stable. Contrast pooling on imaging means active bleeding and the failure rate of non-operative management is high. Here, we describe a 65 year-old woman who sustained concomitant blunt hepatic and splenic injuries with simultaneous contrast pooling in both organs. The patient survived after angiographic embolization of vascular lesions of both solid organs.

Selective Angiographic Embolization of Blunt Hepatic Trauma Reduces Failure Rate of Nonoperative Therapy and Incidence of Post-Traumatic Complications

BACKGROUND Conflict still remains as to the benefit of angioembolization (AE) for non-operative therapy (NOT) of blunt hepatic trauma (BHT). The aim of this study was to determine whether AE could result in lower failure rates in hemodynamically stable BHT patients with high failure risk factors for NOT, and to systematically evaluate the effectiveness of AE for NOT of BHT. MATERIAL AND METHODS...

Nonoperative management of blunt splenic injury: a 5-year experience.

OBJECTIVES The purpose of this study was to examine the success rate of nonoperative management of blunt splenic injury in an institution using splenic embolization. METHODS We conducted a retrospective review of all patients admitted to a Level I trauma center with blunt splenic injury. Data review included patient demographics, computed tomographic (CT) scan results, management technique, a...

An alternative option in the management of blunt splenic injury

Splenic injury is a preventable cause of mortality following blunt trauma. The majority of splenic injuries can be managed conservatively. Laparotomy is indicated in the haemodynamically unstable patient, or those with other intra-abdominal injuries requiring surgery. Angio-embolization can be used to achieve haemostasis and preserve splenic parenchyma. The expertise and experience of the multi...

Concomitant injuries are an important determinant of outcome of high-grade blunt hepatic trauma.

BACKGROUND Little is known about the clinical importance of concomitant injuries in polytraumatized patients with high-grade blunt liver injury. A retrospective single-centre study was performed to investigate the safety of non-operative management of liver injury and the impact of concomitant intra- and extra-abdominal injuries on clinical outcome. METHODS Some 183 patients with blunt liver ...

Laparoscopic Splenectomy Following Embolization for Blunt Trauma

BACKGROUND Grade III through V splenic injuries as defined by the American Association for the Surgery of Trauma (AAST) grading scale are associated with hemorrhage and nonoperative failure. Embolization has been reported to reduce splenic bleeding in 50% to 75% of patients with a high-grade injury. However, splenectomy following embolization may be necessary in patients who continue to bleed o...
